All paddlers, especially youth and Novice paddlers, for their own safety, will be required to pass a swimming test before they can participate in paddling activities.
I understand that if I willfully and knowingly misrepresent my paddling/racing status and participate in a racing event under these circumstances, NHCCM will be fined $300.00 by OHCRA for violating the OHCRA race rules regarding an “Illegal Paddler”. This $300.00 will be charged to my account, and I will not be able to transfer until paid in full. Regatta Members: I understand that club membership/registration fees are payable on or before April 30th, and there will be NO refunds. I affirm that the registration information was answered honestly and in good faith. I will be solely responsible for any/all willful or knowing representations.

In consideration of being allowed to participate in sports, recreational, and related events of any kind, the undersigned releases from liability the Oahu Hawaiian Canoe Racing Association and its member organizations, affiliated clubs, their administrators, directors, agents, coaches,volunteers or employees, other participants,sponsoring agencies, sponsors, advertisers, and owners and leasees of premises where activities or events are conducted, hereinafter referred to as “Releasees,” in full understanding and agreement that: 1. Paddling and other club activities are inherently dangerous, involving risk of serious injury, including permanent disability and death, and severe social and economic losses which might result from the actions, inactions or negligence of their own or of others, the rules of play, or the condition of the premises or of any equipment used. Risks may be unknown, and not reasonably foreseeable. 2. Prior to participating, it is paddler's responsibility to inspect the facilities and equipment to be used, and if they believe anything is unsafe, to immediately advise their coach or supervisor of such condition(s) and refrain from participating. Paddlers will follow all rules and instructions relating to conduct and safety. 3. They understand and assume all risk associated with participating, and release, waive and discharge Releasees from liability for demands, losses or damages on account of injury, including death or damage to property related to participation. 4. Paddling and other club activities are inherently public, taking place on beaches, open water and other locations open to the public. Photographs may be taken by Releasees or by members of the public. I hereby grant Releasees the right to use paddler's photograph or recording in any and all forms of media and in all manners including, but not limited to edited, or composite media formats, for the purpose of documenting the perpetuation of the art of the sport of Hawaiian Outrigger Canoe training, rigging, paddling and racing; for news release, art, advertising, trade or any other lawful purpose whatsoever; and release, discharge, and agree to save Releasees from any liability by virtue of blurring, distortion, or alteration whether intentional or otherwise, that may occur or be produced in the making of said images. The undersigned custodial parent or legal guardian acknowledges that he/she is also signing this release on behalf of the minor participant, that he/she is waiving certain rights on behalf of the minor participant that the minor participant otherwise may have and that the minor participant shall be bound by all of the terms of this release. By signing this waiver and release without a parent’s or guardian’s signature, the participant represents he/she is at least 18 years of age, or, if signing as the parent or guardian of the participant, signer represents they are the custodial parent or legal guardian of the minor participant. THE UNDERSIGNED HAVING READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT THEY HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.
New Hope Canoe Club Ministry Waiver Plain Language Release from Legal Liability for Injuries The undersigned, hereinafter referred to as “Releaser”, in consideration of being permitted to participate with the New Hope Canoe Club, do hereby release and hold harmless the Ministry, New Hope O`ahu from any and every claim, demand, action, or right of action of whatever kind or nature arising from damage and/or personal injury resulting from any accident, health condition, illness, contagious virus or disease which may arise from the Releaser's participation in the ministry or any activities in connection with a function, caused by the negligence or any other act or conduct of the chapter elder and members of the New Hope New Hope Canoe Club Ministry, or any employee, officer, or director of New Hope O`ahu. I release, waive, discharge and covenant not to sue New Hope Canoe Club, New Hope O’ahu, its member associations, its affiliated ministries, their respective administrators, directors, agents, chapter elders and other volunteers or employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “releases”, from demands, losses, or damages on account of injury, disability, including death or damage to property, caused or alleged to be caused in whole or part by the negligence of the release or otherwise. I further release all members and chapter elders from any claim whatsoever I may have on account of first aid, treatment or services rendered or not rendered to me during my participation in this ministry. I agree to submit a doctor’s note clearing me to participate, if I have a medical condition which may threaten my life or the other participants. I understand that my participation in this ministry may be denied, if the chapter elder and staff determines that by doing so, it may be hazardous to my health or to the other participants, or if my physical conditioning is not at the level it should be at to participate in practices, games or events with the New Hope Canoe Club Ministry. I understand that being a member of the New Hope Canoe Club Ministry does not guarantee my participation in practices, games or events, but it is determined by the ministry leaders and staff evaluation of my ability to do so. This release contains the entire agreement between the New Hope Canoe Club Ministry, New Hope O’ahu and me, and the terms of this release are a contract. I have read this release, I understand it, and I sign it as my own free act. I understand that before I sign this, I may have my attorney review this release and he or she can explain in detail the full extent of the legal rights I am giving up. I warrant that I am an adult paddler, over the age of 18, or that I am the legal guardian/parent of the minor child applying for membership, for whom I am waiving rights.

NHCC VOLUNTEER SIGN UP FORM New Hope Canoe Club is strictly a volunteer based and driven canoe club. We are also self sufficient, which simply means we do not get any funding from any outside agency or company. However, we do fundraising events such as sweet bread/chili sales. Our major fundraiser is our Malama Mokauea Pre Season Race. It is a huge event and we will need everyone’s help. We ask each member to get involved somewhere in our canoe club by signing up for a committee. Whether it is helping with equipment, assisting with registration, fundraising, or helping to assist our coaches, we encourage each member to get involved according to their gifts and talents. If you are unsure of your gifts and interests, please feel free to jump in anywhere and learn as you go. We want all of our members to excel in the position of their choosing, and to grow in their God-given gifts. Please fill out the form below and turn it in with your registration forms.
